The Housing Coordinator is responsible for overseeing and coordinating the implementation of the HOPWA program to help assist and promote housing stability to persons living with HIV/AIDS. The Housing Coordinator will also provide case management services to persons living with HIV/AIDS as directed by policies and procedures of the HOPE program and associated funding agencies. JOB DUTIES AND RESPONSIBILITIES: • Triage and assess all information and referral inquiries to determine eligibility for Housing Assistance for person’s living with HIV/ AIDS in the HOPE Program. Housing assistance activities include but are not limited to housing assessments, screenings, preparation of housing service coordination plans, housing referrals and placements, housing advocacy. • Adheres to state mandated regulations per Pennsylvania DOH by maintaining and updating revisions to the HOPWA Manual to assure compliance. • Attends available Housing Training Opportunities as agency budget allows or as directed. • Works in collaboration with HOPE Epidemiologist and AIDSNET on the HOPWA Consolidated Annual Performance and Evaluation Report (CAPER) and other reports/documents that may be requested. • Maintains accurate Housing records in approved formats in order to assure full compliance with funders and state regulations. • Functions as a professional Housing Coordinator and resource, providing guidance to co-workers and HOPE team members. • Demonstrates competency in housing assessments, housing eligibility, and regulations from the Pennsylvania DOH and the Housing and Urban Development (HUD). • Demonstrates financial responsibility and accountability in all housing files through effective and efficient use of housing resources in daily procedures, processes and practices. • Maintain regular communications with landlords, various community liaisons, funders, and accounting department personnel. • Responsible for developing and implementing housing policies and procedures in collaboration with Supervisor, HOPE Program Case Management. PHYSICAL AND SENSORY REQUIREMENTS: Sitting for up to 4 hours per day, up to 2 hours of time. Standing for up to 6 hours per day, 3 hours at a time. Walking for up to 6 hours per day, 20 minutes at a time. Frequent use of hands and fingers. Consistently lift, carry, and push of objects up to 25lbs. Frequent stooping and bending. Frequent reaching above shoulder level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, and high and low frequencies. Must be able to see as it relates to general, near, far, color, and peripheral vision. EDUCATION: Bachelor's degree in social work, psychology, sociology or related field required. TRAINING AND EXPERIENCE: Five years of experience in case management services required. Two years of experience in supportive housing services for low income individuals required. One year experience in HIV/AIDS management and assessment techniques preferred. Knowledge of HIV risk behaviors and behavior change principles preferred. Bilingual in English and Spanish preferred.
