Licensed Vocational Nurse (LVN/LPN) Role: Making a Difference in Patient Lives

We're seeking an experienced and skilled Licensed Vocational Nurse (LVN/LPN) to join our team at the Round Rock Rehab facility on a full-time basis, working days. As an LVN/LPN, you'll play a vital role in providing appropriate nursing care to our patients under the supervision of a Registered Nurse (RN). Your diverse skillset and experience will be essential in driving success in this position.

Key Responsibilities

• Patient Care: Assume responsibility for the care of assigned patients on designated shifts, administering oral and IV medications as prescribed, charting appropriately and timely, reviewing and updating care plans, taking and recording vital signs, and providing ostomy care.
• Monitoring and Reporting: Monitor patients for changes in medical condition, notifying physicians as necessary, changing wound dressings, collecting specimens, and inserting and caring for nasogastric tubes, urinary catheters, and tracheostomy/ventilator care.
• Reporting and Documentation: Collaborate in the development of the plan of care, assess the need for community-based resources, observe patient progress, and report changes in patient status to the RN and physician.
• Effective Communication: Utilize effective communication skills to report changes and document in the medical record, accurately recording vital signs, intake and output, and other physical observations.
• Supporting the RN: Assist the RN in obtaining physician orders to meet the medical and nursing needs of the patient.

Essential Qualifications

• Current licensure as a Licensed Practical Nurse/Licensed Vocational Nurse in the state where the hospital resides.
• Current BLS certification.
• Verbal ability to effectively express ideas and views for communication with patients, visitors, and staff members.
• Ability to read and write English language, including medical terminology and abbreviations.
• Clerical ability to document completely and accurately in the medical record.
• Cognitive ability to implement the nursing process with patients in the skilled nursing setting.
• Organizational ability to complete nursing tasks in a timely and efficient manner.

Preferred Qualifications

• Prior experience in emergency or critical care environments.
• Higher comfort level working with vents/trachs/PICC lines, etc.
• 2 years of experience in direct patient care.
• One year of inpatient medical-surgical nursing experience.
