RN Navigator- Cardiovascular Services
Job Description: Serves as a patient liaison to help patients and family members navigate the complex healthcare system. Functions as a clinical resource to patients, physicians, and the assigned clinic site(s) through all phases of trajectory by coordinating a system-wide delivery of service expectations. Practice Details: The Shift for this position is 8am-5pm- MST This is currently an onsite position, and will travel to care sites. Remote work may be an option after a min of 6-8 months of training with our clinics, physicians, and care sites. 1. Evaluate a patient to receive care and assist with urgent needs (trigger words for RN triage call as opposed to normal telephone encounter call initiative) and navigates non-established patients to the correct clinic and provider until scheduled. 2. Utilizes the nursing process to provide coordinated patient care by actively collaborating with other members of the healthcare team and case managing patients. Assesses clinical, emotional, spiritual, psychosocial, financial and other patient needs. 3. Provides education to patients for procedures and testing as needed and follow-up with patients following their procedures. 4. Liaison between doctor of the day/specific providers as it relates to assistance w/other provider inquiries, Peer-to-Peer calls, etc. 5. Provides guidance/recommendations to providers regarding patient health status and address patients urgency for same day or next day add on. 6. The ability to assist Patient Service Reps for urgent scheduling of various testing (prioritize timing and location) and patients with urgent device findings to schedule them in a timely manner. 7. Assist with in-clinic testing as needed to coordinate with doctor of the day to determine next steps in care. 8. Helps coordinate advanced imaging to make sure all components necessary for test are completed. 9. Promotes mission, vision, and values of SCL Health, and abides by service behavior standards. 10. Performs other duties as assigned.
