Remote Case Manager Assistant
About the position Responsibilities • Assist case management team in day-to-day activities, including scheduling, appointment reminders, and maintaining electronic files. • Support case managers in collaborating with healthcare teams, participants, and caregivers to provide outstanding care. • Work with participants and healthcare professionals to identify resources and community-based agents for optimal care. • Collaborate with the case management team to ensure treatment progression and measurable outcomes. • Identify barriers to care and participant engagement, addressing these to prevent suboptimal outcomes. • Facilitate communication and care coordination among healthcare team members and involve participants in decision-making. • Assist case managers with coordinating care interventions, including referrals to community support services. • Develop collaborative relationships with stakeholders and provide support and resources for participants. • Assist with documenting clinical case management and psychosocial services effectiveness. • Coordinate life skills education and support for participants. • Maintain ongoing communication with participants, families, and healthcare professionals regarding care plans. • Ensure appropriate allocation and coordination of healthcare services while improving safety and quality of care. • Educate participants and families about treatment options, community resources, and case management services. • Assist with completing notifications and pre-authorizations for services based on payer requirements. • Support the safe transitioning of care for participants to appropriate levels and providers. • Promote participant self-advocacy, independence, and culturally-appropriate care. • Advocate for participants and payers to facilitate positive outcomes. • Assist with evaluating the effectiveness of case management plans and resource allocation. • Notify team and stakeholders of any negative incidents within 24 hours and maintain accurate documentation of meetings. • Gather and organize therapy notes for monthly uploads. Requirements • Bachelor's degree or AAS degree in health care, nursing, counseling, social work, or psychology. • Two years of experience in case management within a social service or employment counseling position. • Experience working with brain injury participants preferred. • Demonstrable knowledge and advocacy for individuals with brain injury issues. • Proficient in Word, PowerPoint, Excel, SharePoint, and Outlook programs. Nice-to-haves • Experience performing outreach and making referrals for services. • Broad working knowledge of local services. • Excellent interpersonal skills with colleagues and community leaders. Benefits • Competitive hourly wage of $22 to $28 based on experience.
