We are seeking a highly motivated and experienced individual who shares our passion for advocating for underserved communities and addressing social determinants of health. As a key member of our team, you will develop and implement community outreach and partnership initiatives, collaborate with local organizations and stakeholders, and drive our efforts to promote health equity.
Key Responsibilities:
- Develop and implement community outreach and partnership initiatives to address social determinants of health and improve health outcomes
- Collaborate with local organizations and stakeholders to identify health disparities and develop targeted strategies to promote health equity
- Conduct research and analysis to identify key issues and trends related to social determinants of health
- Develop and maintain relationships with community partners to ensure effective collaboration and maximize impact
- Create and deliver presentations and trainings to educate stakeholders on social determinants of health and strategies for addressing them
- Monitor and evaluate the effectiveness of community initiatives and make recommendations for improvement
- Stay current on national and local policies and initiatives related to health equity and social determinants of health
- Represent CVS Health at community events and meetings to raise awareness and advocate for health equity
- Develop and manage budgets for community outreach and partnership initiatives
- Collaborate with internal teams to integrate social determinants of health strategies into company-wide initiatives and programs
- Foster a culture of diversity, inclusion, and cultural competency within the organization
- Serve as a subject matter expert on social determinants of health for the organization and provide guidance and support to colleagues
- Act as a liaison between CVS Health and community organizations, communicating our goals and initiatives and identifying opportunities for collaboration
- Stay informed on best practices and emerging trends in addressing social determinants of health to continuously improve our approach
- Serve as a champion for health equity and advocate for underserved communities within the organization and externally
