Key Responsibilities
- Review and analyze incoming patient data, including vital signs, symptoms, and other relevant health indicators, to identify any critical or out-of-range readings.
- Promptly address any critical or out-of-range readings and escalate as necessary according to established protocols.
- Monitor patients' vital signs and health data remotely using specialized monitoring equipment and software.
- Communicate with patients via phone to discuss their health status, provide guidance, and answer questions.
- Explain medical terminology and procedures in an understandable manner, ensuring patients are informed and engaged in their healthcare management.
- Address patient concerns and questions promptly, providing reassurance and guidance where necessary.
- Manage a significant volume of patient calls within established timelines.
- Work towards achieving personal and team performance metrics.

Requirements and Qualifications
To be successful in this role, you must have:
- An active Compact LVN/LPN license.
