Key Responsibilities:
- Deliver professional nursing care to patients in accordance with established plans of care
- Conduct comprehensive assessments, develop individualized plans of care, and provide skilled interventions to achieve realistic outcomes
- Ensure compliance with federal and state regulations governing hospice care services
- Provide education and support to patients, families, and caregivers
- Collaborate with our clinical team to achieve exceptional patient outcomes and experiences
- Maintain accurate and thorough documentation of patient care and progress

Requirements:
- Graduate of an accredited school of nursing, Bachelor's degree preferred
- Current Registered Nurse licensure (in good standing) in the state(s) of practice
- One (1) year of experience as a nurse in hospice care or post-acute setting
- Current CPR certification and valid driver's license
- Strong clinical skills, excellent communication and interpersonal skills, and ability to work in a fast-paced environment
