Key Responsibilities
In this role, you will be responsible for:
- Researching available options and resources for members with financial, transportation, safety, and housing needs, spanning the entire lifecycle.
- Empowering members to make positive lifestyle and medical choices through telephonic or electronic exchanges.
- Working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
- Utilizing your expertise to navigate complex cases and provide support to members in need.
- Maintaining accurate and detailed records of member interactions and interventions.
Essential Qualifications
To be successful in this role, you must possess:
- An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in your state of residence.
- A minimum of 2+ years of experience in medical social work or case management.
- A Master's Degree in Social Work.
