About the Role

We are seeking an experienced and compassionate Hospice Case Manager - RN to join our team in Avon, IN. As a member of our clinical team, you will lead by example and deliver professional nursing care to our patients in accordance with established plans of care. Your expertise and passion for delivering high-quality care will be essential in ensuring that our patients receive the best possible support and services.

Key Responsibilities

• Deliver professional nursing care to patients in accordance with established plans of care, ensuring quality and compliance with all relevant professional standards and state and federal requirements.
• Perform comprehensive subjective and objective assessments of patient status, including physical, psychosocial, and environmental parameters.
• Formulate individualized plans of care that incorporate analysis of assessment data and modify and update as needed.
• Provide skilled interventions aimed at achieving realistic outcomes within specified time periods, ensuring that interventions are performed within the nursing scope of practice.
• Assure thorough and accurate documentation with all clinical data, including admission through discharge, physician's orders, skilled nursing visits, and changes in conditions.
• Consistently demonstrate competency with technical nursing skills according to the legal scope of practice and implement teaching specific to patient/family needs.
• Evaluate and modify patient care plans as needed, performing clinical chart record reviews and on-call responsibilities as assigned.
• Determine if patients meet admission criteria and assist in the assignment of qualified personnel, preparing clinical progress notes and reviewing documentation on a continual basis.

Essential Qualifications

• Current RN licensure in the state of Indiana.
• Minimum of 2 years of experience in hospice care or a related field.
• Strong knowledge of hospice regulations and standards of practice.
• Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
• Strong assessment and problem-solving skills, with the ability to prioritize and manage multiple tasks and responsibilities.

Preferred Qualifications

• Experience with electronic health records (EHRs) and proficiency in using technology to support patient care.
• Certification in hospice and palliative care (CHPLN or CHPN).
• Experience in case management and care coordination.
• Strong leadership and mentoring skills, with the ability to precept new staff and provide orientation and training to the care team.
