Referral source interactions include, but are not limited to working closely with referring hospitals, physicians, board and care facilities, skilled nursing facilities, patients, families, and the general public. Primary clinical responsibilities include receiving/processing referrals and ensuring that admissions occur in a timely and consistent manner. This position enters referral information into the electronic medical records and creates initial orders to facilitate the start of care. Frequently interacts with referral sources, other members of the referral center staff, home health, advanced illness management and hospice staff across offices and all others necessary to facilitate the start of patient care.
